You have reached this page because you are requesting funds from HAA. There are two ways to request a reimbursement. Before completing either, ensure that you have a copy of your receipts/invoice.
1) If you utilize Google, then complete this Google form: Reimbursement Request Form.
2) Complete this form for a fillable text box PDF and email it to the Treasurer.
Note: Any money spent beyond the approved budget for the Office must be approved by the Executive Board.
